Dear Mr Adeyemi,

Re: Mrs Rosa Delgado, DOB 30/01/1955

I am writing to transfer the care of Mrs Delgado, a 71-year-old woman who had an ischaemic stroke on 2 November, to your unit today for daily physiotherapy and occupational therapy and ongoing management of her swallowing difficulty.

Mrs Delgado was admitted with sudden weakness of her right arm and leg and slurred speech. Imaging confirmed a left middle cerebral artery stroke. She arrived too late for thrombolysis and was started on clopidogrel and atorvastatin, and her full medication chart accompanies her. Her type 2 diabetes is diet controlled, with blood glucose between 6 and 9 mmol/L.

Her right arm remains weak, but her right leg is improving. She transfers with the help of one person and walks 10 metres with a frame and one assist. She has mild word-finding difficulty but follows commands, and she is continent. She lives alone, and her daughter visits daily.

I would be grateful if you could continue her soft diet and mildly thick fluids, as recommended by the speech therapist, and arrange a weekly swallow review. She is due at the stroke clinic on 15 December.

Yours sincerely,

Registered Nurse
